How to reinstate your license in Idaho

  1. If your driving privileges have been suspended by the Idaho Transportation Department’s (ITD) Division of Motor Vehicles (DMV) or a judge you may be eligible for a Restricted Driving Permit. You are able to check your driver license status online.
  2. To apply for a restricted permit complete the Restricted Driving Permit Application and mail to Idaho Transportation Department DMV Operations—Restricted Permits PO Box 34 Boise, ID 83731-0034, email to RDPermits@itd.idaho.gov or fax to (208) 287-3880.
  3. If required, file proof of financial responsibility (SR-22) which can be obtained from your auto insurance provider.
  4. Call 1-833-545-0368 to schedule an ignition interlock installation appointment.
  5. Get the ignition interlock device installed, please make sure to bring all required documents to your appointment.
  6. Pay reinstatement fees online and all other required fees.
